How did Alessia make a custom face template?
I would love to know more about manual setting of face templates....
Does this mean I could mocap a hand with a fake face template?

Officially, no. Unofficially, maybe, give it a try.
Custom templates are possible to build in expression through the template wizard. What you essentially have to do is define the root bone (head markers), and then build individual bindings between the markers, select their elasticity with relation to eachother, and then you have the option to name the individual markers. I would be more than happy to walk you through this process to create a custom face template over a remote session. Due to the fact that it is not currently officially supported, I would not be able to walk you through creating one for a hand, however.
