Body reversed when creating skeletons

Hello,

We've been having some trouble creating anatomically-correct skeletons. We're using the 25-marker upper body template, and we get our subjects to make the T-pose while we try to create the skeleton. About 30% of the time, the head and body of the fitted skeleton appear to be facing in opposite directions. Checking the marker labels tells us that the head markers are correct, but the body markers are reversed (i.e., Motive identifies the chest marker as the top back marker, and the left and right sides of the body are reversed).

We've experimented quite a bit with this (by slightly repositioning the markers and/or modifying the subject's T-pose) but have not been able to figure out what triggers this particular fitting error. Is this a common problem and does anyone know what causes it? I assume we are consistently making a novice error with our marker placement, but we can't figure out what it is.
