Head Offset When Creating Skeleton

When I create a skeleton, the head will be aimed a bit down and to the left, as if there is an offset from what would be expected from the actor's posture.
In the attached image, the forehead marker looks like it was placed above the actor's right eye, when it is actually placed in the center of his forehead.
I've tried experimenting with marker placement on the head, and the "Head Upright" setting, but I cannot seem to correct the issue. Any suggestions for how to solve this? Are there best practices for head marker placement?
