IR4 Dead No Lights Nothing

IR4 purchased from yourselves February 2007, has stopped working,I have downloaded and installed 4.1.030 B final, also NPRegfix.exe, still nothing. Changed USB ports and plugged into another machine. I have read other post where you advised user to return to supplier for replacement. Is this a known fault?
I think the serial number which I found on a Bar Coded lablel on the USB cable is = 0xxxxxx
