Ok, updated to the 1.12b patch, and all I managed to do with start the game, click on the options button and that was it. I heard the windows failure sound, alt-tabbed to the desktop and was greated with the error message. It doesn't like 1.12b.
Does anyone have it actually working with that version?
Not sure if I mentioned it before, but I'm using an 8800GTX with the latest drivers. Chipset and sound drivers are also the latest.

Thanks, I've almost given up with this now as its just plain annoying. I don't want to go back to 1.11 as I won't be able to do multiplayer with the rest of my squad

Ok, I somehow managed to get it to work, but I did a load of crazy stuff I'm not to sure about! I'm going to try and replicate it tonight and then post it up.

Right, this is what I did, and this is with the latest LOFC patch:
I uninstalled TrackIR completely, and I deleted the Naturalpoint folder in Program Files too.
I then went to the LockOn folder which is in the Ubisoft folder under Program Files and deleted the TrackIR files from both the Config and Input folders. (also removed same file from EnglishVersion folder that was in the Input Folder)
This is where it breaks off, and I haven't tried this with the latest patch yet... I reinstalled TrackIR from version 4.1.030 final B.
Once this was done I simply loaded up LOFC and it has worked without a problem. Several reboots later and all ok.
I should try it with the latest TrackIR software but as I suspected it was something that was caused by the latest LockOn patch I looked for a version of the TrackIR software that predated the new patch, as I assumed the guys who made it would have checked the version of TrackIR that was available at the time of the 1.12 patch release would function ok.

There would have been nothing that changed within the TrackIR software as it relates to the gaming application.

When I try the same procedure with the latest version of TrackIR we will see just what its up to. I'll give it a go over the weekend and repost my results.
I have had it go back to the same error yesterday after a GFX driver update which I will admit didn't surprise me one bit.

Ok, tried it with the latest Nvidia GFX driver, latest version of Lockon and latest TrackIR driver/software and it did the exact same thing as before. You only need to go to the options screen to get the fault, no need to start a quick mission or anything.
I'm just going to try all the different drivers until I get something stable.

I'm curious if there's been any sort of fix to this issue.
I just fired up Lock-On (1.12b) and I'm having this exact problem. The mission load screen gets about 1/3 through the progress bar, I hear a system error beep and TrackIR has locked up. I've tried all the listed fixes; deleting files and whatnot.
On occasion, after deleting the config file, I can get into a mission (even when the system beep lets me know TIR has crashed) and the tracking software will work for that ONE mission. Should I drop out and start a new mission, TIR ceases to function.
When I ALT-TAB out I can see a small dialog up from TIR which says "unhandled exception" with a non-functioning "OK" button. Sometimes the dialog has no text showing at all. In all cases TrackIR is so locked-up I have to kill it with the task manager.
FYI, I'm running XP Home SP2, all the latest and greatest patches. Hardware is pretty stock; 3Ghz Core 2 Duo, 8800GTX, onboard sound, windows drivers for joystick,...nothing exotic (besides TIR).
I'd hate to do a "Format C:/" but I'm not sure what's going on here.
