Only two green lights showing

I find when running TrackIR5 that I am only seeing two green lights, the third on the top is red. I have tried moving my head around and sometimes it will turn green but never stays green. Is this a fault with the transmitter or the reflectors on my cap?

Would you be able to load up a screen shot of what it is doing? What are your light filter threshold and IR brightness set at?
